MDOC | Patient's Obligations

★ To provide all personal and family health information (medical and dental & orthodontic history) needed to provide you with appropriate care.
★ To participate to the best in making decision about dental treatment plan.
★ Responsibility to follow instruction and comply with the agreed plan of treatment.
★ To accept the consequences of recommendations and refusing treatment.
★ To ask questions to doctor or other care provider if there is desire to transfer of care to another doctor, care provider or facility.
★ To show respect and consideration of others receiving and providing care and to be courteous to staff.
★ To observe facility policies and procedures regarding smoking, noise and caring of the centre belonging.
★ To use the emergency and walk in services appropriately.
★ To inform the medical staff if they have any special requirement regarding their dental treatment or
any administrative staff (Front Office Manager) regarding other non-medical issues (insurance form, using Center facilities, etc.
★ To be punctual regarding appointments and inform front office staff if they are unable to attend at least 1 to 3 days before
so that the appointment may be used for someone else in need & scheduled on center waiting list .
